She wants to get licensed to use her medical degree here in the US - not necessarily as a gynecologist but definitely as an MD. If people have advanced degrees and good language skills AND get their credentials evaluated, they may be able to get an employer to sponsor them for an H1 B visa otherwise doing this without workauthorizaiton, they will not get hired. I have been in Human Rsources for over 20 years in aa variety of industires
